Identification: Discuss how the problem is identified (screening, diagnosticissues, questionnaires, etc.)d. Interventions: Give a brief overview of strategies or interventions forindividuals, families or communities that have been tested, along with anybarriers or things that may interfere with addressing the problem,supported with evidence-based research.e. Discuss one of the health behavior change theories or models ortheoretical frameworks that is relevant for how people might change theirbehaviors to address your topic and describe how it has been used in theliterature (positive or negative) in addressing your topic.f. Conclusion: Wrap up the paper topic and discuss what is needed next toimprove the health related to your topic.3. Use APA format for the entire paper, reference list, and for reference citationsin the text. You must cite the sources for information you put in the paper. Youshould have a minimum of 10 relevant references (not counting thetextbooks). References should have been published in the past 5 years. Websites should only be used if they provide key information (for example, the CDChas some statistics on health problems, guidelines from the USPSTF or otherprofessional source may be cited, etc.).4. The paper length should have a cover page, 10-12 pages of text (doublespaced), followed by references. The font should be 12 point, and the margins allaround the paper should be 1. Quotes should NOT be used at the graduatelevel you should be summarizing the literature and paraphrasing what othersdiscuss rather than using quotes (rarely a specific definition may be needed andthen provided in quotes).5. After your references, attach a list of 8-10 community and published resourcesfor health care professionals and patients. This could include some web sites,but you must note the specific page that is relevant to your topic (i.e. not just thecdc.gov web site).6.
